First Contact Physiotherapists and Mental Health Practitioners work alongside our existing practice team to provide enhanced access to health and social care services within a primary care setting for all of our patients. Please find some information below about the services they provide:
Mental Health Practitioner
As part of the new practice based multi-disciplinary team, our GP Practice now has a Mental Health Practitioner available to registered patients of all ages.

First Contact Physiotherapist

Social Work
The Community Support Social Work Team have now joined the multi-disciplinary team within Willow Medical Practice. Your social wellbeing affects how you feel about yourself, how well you function and the overall quality of your life. Issues which affect your social wellbeing may have a significant impact on your physical or mental health. Our Community Support Social Work Team can discuss any problems you may have and explore what is important to you. They will provide you with practical advice, help and support in order to address these problems.
Our Community Support Social Work Team can support you on a number of issues, including but limited to:
- Housing difficulties
- Behaviour Management/Autism/ADHD
- Bereavement
- Alcohol/drug issues
- Learning Difficulties
- Isolation or loneliness
- Carer support
- Relationship difficulties
- Parenting difficulties (including individual work with children / young people)
- Stress
- Traumatic life events
- Financial difficulties
- Feelings of anxiety
- Self-Harm
- Mental Health (community support)
- Domestic Abuse
- Anger Management
- Referrals to Social Prescribing
- Access to services/signposting
Where the social worker feels you may benefit from other services, they may work with other people or groups in your local community that can support your needs.
